Эффект техники освобождения при помощи давления на латентную миофасциальную триггерную точку в верхней части трапециевидной мышцы: рандомизированное контролируемое клиническое исследование

Аннотация

Введение. Латентные миофасциальные триггерные точки (ЛМТТ) являются причиной интенсивной и спонтанной боли. При нетравматической механической боли в шее надавливание в области шейного отдела позвоночника вызывает гипералгезию и оказывает влияние на мышцы шеи, такие как верхние части трапециевидных мышц, поскольку они получают иннервацию от уровня CII–IV.
Цель исследования — узнать, как влияет техника освобождения при помощи надавливания на ЛМТТ в верхней части трапециевидной мышцы на болевой порог при надавливании (БПН) в этой мышце, в мышце, поднимающей лопатку, большом затылочном нерве (БЗН), надглазничном нерве (V1) и суставных отростках CIII–IV с обеих сторон, а также на диапазон активных движений шеи (ДАДШ).
Материалы и методы. В исследовании приняли участие 60 человек, имеющих ЛМТТ в передневерхних волокнах верхней трапециевидной мышцы. Значения БПН измеряли с двух сторон в верхней трапеции, мышце, поднимающей лопатку, БЗН, V1 и суставных отростках CIII–IV с помощью цифрового альгометра. ДАДШ при флексии, экстензии, гомолатеральной боковой флексии, контралатеральной боковой флексии, гомолатеральной и контралатеральной ротации измеряли с помощью пузырькового инклинометра. Основной группе была выполнена техника освобождения миофасциальной триггерной точки, а контрольной группе – техника плацебо с ультразвуком без интенсивности.
Результаты. Полученные результаты показывают, что техника освобождения при помощи надавливания на ЛМТТ в верхней трапециевидной мышце является эффективной и проявляется увеличением БПН в этой мышце сразу после ее выполнения (p<0,01). Также было подтверждено, что увеличение БПН в верхней трапециевидной мышце после освобождения ЛМТТ сопровождалось одновременным увеличением БПН в контралатеральной верхней трапециевидной мышце, в мышце, поднимающей лопатку с двух сторон, БЗН билатерально, суставных отростках CIII–IV билатерально и V1 гомолатерально. В обеих группах наблюдали значимое увеличение ДАДШ (p<0,01). Однако в основной группе это увеличение было значительно больше, что с клинической точки зрения может быть весьма полезным.
Выводы. Техника освобождения при помощи надавливания на ЛМТТ в верхней трапециевидной мышце является полезной в остеопатии, поскольку вызывает увеличение БПН и ДАДШ сразу после ее применения.
