Введение. Идиопатическая невропатия лицевого нерва (ИНЛН) — код G51.0 по МКБ-10 — является наиболее частой патологией при поражении лицевого нерва, достигая 70 %. Заболевание характеризуется острым развитием в течение 24–72 ч односторонней слабости верхней и нижней порции мимических мышц, лагофтальмом и, в зависимости от уровня поражения ксерофтальмией или лакримацией, нарушением вкуса, гиперакузией.Цель исследования — разработать и оценить эффективность интегративного подхода по сравнению со стандартной терапией при лечении ИНЛН.Материалы и методы. Были обследованы и пролечены 35 пациентов, которые были распределены на нерандомизированные группы: основная (n=17) — лечение включало локальную инъекционную терапию, тейпирование лица, остеопатию, акупунктуру и ЛФК; контрольная (n=18) — общепринятое лечение согласно клиническим рекомендациям (преднизолон, ацикловир, витамины группы В, тиоктовая кислота, легкий массаж, ЛФК).Результаты. После лечения в основной группе было отмечено более быстрое наступление клинически значимого улучшения, проявляющегося полным устранением или существенным уменьшением явлений прозопареза. Степень нарушения функции лицевого нерва оценивали при помощи шкалы Хауса–Бракманна в баллах. При оценке данных основной и контрольной групп через 1 мес после лечения была получена статистически значимая разница — 1,24±0,14 и 1,89±0,16 балла соответственно (p=0,004), а также статистически значимая разница через полгода — 1,12±0,08 и 1,56±0,15 балла соответственно (p=0,014).Заключение. Комплексный подход с применением локальной инъекционной терапии, тейпирования, акупунктуры, остеопатии и ЛФК эффективен при лечении идиопатической невропатии лицевого нерва, позволяет быстрее добиться клинического улучшения, снизить риски нежелательных явлений медикаментозной терапии и развитие осложнений. Это позволяет рекомендовать его для более широкого использования в клинической практике у данной категории пациентов.
Introduction. Idiopathic facial neuropathy (IFN), ICD-10 code G51.0, is the most common pathology affecting the facial nerve, accounting for up to 70% of cases. The disease is characterized by the acute development within 24 to 72 hours of unilateral weakness of the upper and lower facial muscles, lagophthalmos, and, depending on the severity of the lesion, xerophthalmia or lacrima, taste disturbance, and hyperacusis.The aim of the study: to develop and evaluate the effectiveness of an integrative approach compared with standard therapy in the treatment of IFN.Materials and methods. A total of 35 patients were examined and treated and divided into non-randomized groups: the main group (17 people), whose treatment included local injection therapy, facial taping, osteopathy, acupuncture and exercise therapy, and the control group (18 people), with generally accepted treatment according to clinical guidelines — prednisolone, acyclovir, B vitamins, thioctic acid, light massage, exercise therapy.Results. Following treatment, the main group demonstrated a more rapid onset of clinically significant improvement, manifested by the complete elimination or significant reduction of prosoparesis symptoms. The degree of facial nerve dysfunction was assessed using the House–Brackmann scale in points. When evaluating the scores after treatment in the main and control groups, a statistically significant difference of 1,24±0,14 and 1,89±0,16, respectively (p=0,004) was obtained, and after six months — a statistically significant difference of 1,12±0,08 and 1,56±0,15, respectively (p=0,014).Conclusion. A comprehensive approach to treating idiopathic facial neuropathy, using local injection therapy, taping, acupuncture, osteopathy, and exercise therapy, is effective, allowing for faster clinical improvement, reducing the risk of adverse drug reactions and the development of complications such as contractures and synkinesis. This allows us to recommend it for wider clinical use in this patient population.
